MS Care Management
Connecting people with MS to services and support.
Inglis’ MS Care Management program assists individuals with multiple sclerosis to find the necessary supports and services for their individual needs so they can live independently in the community.
Through our partnerships with the Jefferson Comprehensive Multiple Sclerosis Center and the National Multiple Sclerosis Society, our Care Managers assess and support people who require help as their needs change. Additionally, Care Managers visit individuals in their homes for assessments and follow-ups.
The Care Managers provide individualized and flexible services that may also connect individuals to many other community services offered through Inglis (and others) focusing on independence, health and wellness, socialization, education, and job support.
Services may include:
- Financial support
- Assistive technology resources
- Connection to Community HealthChoices
- Managing home health services
- Home modifications
